Medicaid
For patients with Medicaid (also known as OHP), we accept those being covered by CareOregon, Providence dual Medicare and Medicaid, Trillium, PacificSource, and some OHSU.
We do not accept Kaiser Medicaid or Providence Medicaid only.
Medicare
For those with Medicare, you have to meet your deductible and co-insurance obligations before Medicare starts reimbursing us for the services provided to you.
Thus, we would like you to pay a deposit of $223.00. This amount will be applied toward your deductible and co-insurance. At the end of the year, the remainder of the deposit will be refunded to you if it is not used up.
Commercial
We accept many commercial insurance including Regence/Blue Cross Blue Shield, Cigna, Aetna, MODA Health, Health Net, United Health, Providence Health Plan and many more.

We recognize that many with chronic diseases, such as Hypertension, High Cholesterol, Diabetes ,who happen to have high deductible plans. These plans typically have deductibles ranging from $1,000.00 to $6,000.00. Therefore, having these plans means patients pay the monthly premium. Yet every time they go see a doctor, they are responsible for all the costs for getting their chronic problems taken care of. We are ready to help these patients by having them pay a fixed amount yearly that is just a fraction of their deductible plus the cost of any labs that are needed for their optimal care.

Uninsured
For those without insurance, no worries! We are ready to help.
We only charge a $85 fee for an office visit, provided that is not related to any chronic pain.
